
Geriatric Case Manager Kate Sullivan, BSW, joined Cedar Home Health and Hospice in May, 2011 to bring her many years’ experience as a licensed social worker to a growing client list in homes throughout Washington, Dodge, Fond du Lac, Ozaukee, Waukesha and Sheboygan counties.
Sullivan most recently served in the M & I Bank trust department as a geriatric care manager. There, she assisted older adults and their families with not only financial issues, but transported and accompanied clients to appointments, assessed home environments, coordinated moves, helped manage medications, arranged home nursing or supportive care, advocated in health and financial matters, and acted as a liaison in communicating with loved ones. “I see my role at Cedar as helping older adults remain in the living situation that’s best for them,” said Sullivan. “So much of my job is listening to a client’s needs and desires and then applying my knowledge and experience to help him or her succeed.”
A University of Wisconsin – Madison graduate, Sullivan also has experience as a case manager at St. Luke’s Medical Center in Milwaukee. Sullivan said that among the more rewarding aspects of her job has been her ability to sift through new Medicare, Medicaid and Medicare Part D regulations, helping clients take best advantage of benefits, as well as helping save them money by identifying overpayment for cell phones, utilities and other expenses. “I pride myself as being a watchdog for my clients, protecting them from fraud or financial abuse. It’s truly ‘advocacy in action.’” Sullivan said. “What drew me to Cedar’s Pathfinder program is its affordable rates compared to other care providers serving the area. It helps make these vital services more accessible to clients and their families.”
